Our Billing Policy 

We now require a card to be kept on file for payments owed to office:

 

This can be a HSA card, credit card or debit card. You will be asked to sign consent to store the card and charged once patient responsibility is determined by your insurance. 

 

A receipt will be emailed to your email on file. Please notify the office if your payment card needs to be updated.

 

All copayments are due at the time of visit and can not be billed at a later date.

 

PLEASE NOTE: 

Glastonbury Pediatrics, PLLC collects a $10 form fee per form for the completion of all forms other than the CT Health Assessment Record or CT Early Childhood Health Assessment Record. This includes Authorization to Administer medication forms, Asthma and/or Allergy Action plans, FMLA forms, and Disability applications. All form fees will be charged to your stored card and must be paid before the completed form is sent to you. Please allow 3 business days for all form requests to be completed.

We are currently only accepting newborns to the practice.

Our website will be kept up to date and if this changes, it will be posted here. 

Until further notice: We are accepting only newborns, no older children at this time. Please call the office to inquire about how to set up a Meet and Greet call with Dr. Kumar. We offer these calls multiple evenings each week and encourage our families to call early to get scheduled. Please note: we require our newborns to be established with the practice prior to delivery including all necessary paperwork to be submitted. We do not accept patients whose family have not completed our process to become established before birth.

PLEASE NOTE THAT WE DO NOT OFFER DELAYED, STAGGERED OR POSTPONED VACCINES IN THIS OFFICE. 

We are happy to announce we now have a patient portal!

Once your child's chart is active in the portal, you are able to send non-urgent clinical questions during regular office hours, view growth charts, see upcoming appointments, receive forms, and make payments using the portal. Portal messages are retrieved Monday through Friday 8 a.m. through 5 p.m.

​

Please DO NOT send portal messages for appointments or urgent concerns over the weekend. All messages will be responded to during normal office hours only.
